What problem did you address/are you addressing? 
The necessity to adopt any efforts to reduce as much as possible the Accident rate and consequently to improve Road Safety. Moreover the results obtained with this initiative could also help to cut the number of deaths and injuries occurred in our network. The initiative concerns the Accident Investigations with a PDCA process which consists of more stringent analysis criteria in order to intercept, in advance, potential critical issues causing accidents and a system of monthly checks on the effectiveness of interventions that allow re-examining the critical points more quickly.

Name of action 

A continuous implement and upgrading of the tools used, such as the software “AGE”. 
The ASPI classification of “Levels of Accident Rate” considering the number of accidents in the last 12 months along a 1 km long section (five ranges of accidents number: RED ≥ 24; ORANGE 17-23; YELLOW 10-16; GREEN 8-9; WHITE ≤ 8). This classification allows ASPI to quickly identify which sections require a more urgent action. 
A constant analysis of accidents and events occurred over the past few years in order to identify recurrent elements. During this phase the Accidents Data are analysed per year, per each 100 meters, per type of accident (i.e. rear-end impact), per location (i.e. off-ramp), per condition of pavement (dry or wet), per number and type of vehicles involved (heavy and/or light vehicles) etc. in order to identify the recurrent elements and the enablers of the accidents. 
A monthly highways site-visit on those sections analysed, in order to understand in person which elements could lead the users to have wrong behaviours and consequently to be part of an accident. 
A monthly meeting between the TMC of each Regional Headquarters and the TMC of Central Headquarters in order to analyse in depth all the identified sections and to share ideas or actions to be performed.
A constant check of the effectiveness of the actions performed; if the action results are positive, it will become a “best practice” to be applied also in other situations; otherwise, the analysis of accidents will continue to identify other actions in order to reduce the accident rates. 
The distribution of “Best Practices” among all the TMC of Regional Headquarters, in order to apply them to similar situations.

How did you disseminate/are you disseminating results or how did you promote/are you promoting your initiative? 
ASPI has publicised the initiative of PDCA Accident Investigations via events (i.e. ASECAP Road Safety Conference 2017 in Brussels) and publications for Road Safety and Quality of Service (i.e. “Agorà – Idee per la mobilità del futuro” and “Consulta per la Sicuezza e la Qualità del Servizio”). The latter are also accessible to the public on ASPI website.
How did you evaluate/are you evaluating the success of your action? 
The main Road Safety results obtained by ASPI with the PDCA Accident Investigations are:- the decrease of Accidents in PISM by 32% from 2014 to 2016, and by 84% from 2002; - the reduction of number of PISM sections by 27% from 2014 to 2016, and by 82% from 2002.Furthermore this initiative has also helped to achieve the following results:- the reduction of Death Rate (number of deaths per 100 million km travelled) by 81% from 1999 to 2016;- the decrease of Accident Rate (number of accidents per 100 million km travelled) by 54% from 1999 to 2016.
